The Dementia Reablement team provide short term (up to 12 weeks) support to individuals recently diagnosed with dementia. The team provides information, advice and practical support to enable individuals referred to the service to continue to live independently for as long as possible and to enjoy a good quality of life.
An exciting opportunity has become available to work within the Care4CE Dementia Reablement service. We are recruiting a permanent Grade 6 Senior Support Worker for 30 hours per week, over 4 set days, one of which will be a Friday. You will primarily be working between Monday and Friday 9am – 5pm although some early evenings may be required. The service is based in Delamere House, Crewe but covers the south of Cheshire East.

About the Role:
The Dementia Reablement team provide short term (up to 12 weeks) support to individuals recently diagnosed with dementia. The team provides information, advice and practical support to enable individuals referred to the service to continue to live independently for as long as possible and to enjoy a good quality of life. The team provides specialist, short term interventions that are designed to equip people with early dementia, their carers, and their families, with the practical skills, tools and information that can improve outcomes for the person affected.

About you:
You will have:
• Effective teamwork and communication skills
• An enthusiastic, flexible, and reliable approach to work
• Competent computer skills
• Able to work on your own initiative and as part of a team
• Able to manage a busy workload and keep to deadlines
• A clean driving licence with Business Use car insurance required - mileage paid at 52.2p per mile (over 1000cc) or 46.9p (under 999cc/electric)
