Summary

The post holder will be a member of a specialist interdisciplinary rehabilitation team working with patients with acute and long term needs in the community that are neurologically related. The post holder will be expected to provide for the client group by assessing and managing health and rehabilitation needs liaising with primary care, the acute sector, allied health professionals and social care. Health promotion around stroke and the provision of information and education in the community is a significant aspect of the nursing role Main Responsibilities a) To plan, develop, implement and evaluate programmes to promote optimum health and well-being of patients and their carers, including appropriate health assessment tools. To be patient and family driven helping to set and achieve attainable goals b) To carry out nurse specific assessment in community settings; assessing risk to patient and carer; identifying impairments in order to establish a patient centred goal specific rehabilitation programme, including medication management, nutrition and hydration, blood pressure management, psychological care and health promotion c) To offer opportunities to talk about healthier lifestyle options to empower patients and their families to reduce risks of further episodes of illnesses (e.g. hypertension/stroke) d) To carry out initial multi-disciplinary initial assessments and communicate to the team to identify MDT input, needs and risks e) To be jointly responsible for overall day-to-day management of stroke patient referrals f) To maintain accurate and contemporaneous records of care ensuring nursing compliance with NMC, and SCFT standards. g) To undertake supervision and line management of junior staff and students. To provide clinical, informal supervision and training to support workers involving shadowing, joint working input to clinical reasoning and reflective practice as part of the MDT h) To participate in audit activities towards development and effective MDT working using support from line manager i) To demonstrate effective time management skills For more in depth details, please see Job Description attached.
