Job Description Job Title: Pharmacy Technician Location: Abbottswood Medical Centre Reports to: Practice Manager / Clinical Pharmacist Lead / GP Partner Hours: 25 in person, days to be agreed Salary: Band 5 Job Summary The Pharmacy Technician will work as part of the multidisciplinary general practice team to support safe, efficient, and effective medicines optimisation. The post holder will assist in the delivery of prescribing systems, medication reviews, medicines reconciliation, and administrative pharmacy processes to improve patient care and reduce GP workload. The role involves close collaboration with GPs, Clinical Pharmacists, nurses, reception staff and external healthcare providers. Key Responsibilities Medicines Optimisation Support medicines management activities in line with practice and national guidelines Assist with medication reviews under the supervision of a Clinical Pharmacist/GP Conduct medicines reconciliation following hospital discharge and clinic letters Identify and resolve medication queries from patients and staff Promote safe prescribing and cost-effective use of medicines Prescribing Support Process repeat prescriptions in accordance with practice protocols Identify patients requiring structured medication reviews Monitor high-risk medicines and ensure appropriate safety checks Support prescribing audits and quality improvement initiatives Maintain accurate patient medication records Patient Care Provide advice to patients on medicines use and adherence Support patients with medication-related queries Assist with patient education around long-term condition management Signpost patients to appropriate healthcare services Governance & Safety Support implementation of prescribing policies and clinical governance processes Assist in managing medicines safety alerts and recalls Participate in audits and data collection Maintain confidentiality and information governance standards Team Working Work collaboratively within the multidisciplinary team Support workflow optimisation within the practice Attend meetings and training as required Contribute to service development Administration Maintain accurate documentation and records Support stock management where applicable Assist with workflow and prescribing systems Professional Responsibilities Maintain registration with the General Pharmaceutical Council (GPhC) Undertake continuing professional development (CPD) Work within professional and legal frameworks Adhere to practice policies and NHS standards
