Crynodeb
In this role you will be responsible for supporting the practice in delivering excellent patient care by taking a proactive approach to promoting community health and wellbeing. The role will include carrying a case load of patients who you will assist in CGAs, health and wellbeing plans, quality of life referrals (such as the social prescribing service) and encouraging and supporting patients to manage their own health needs. You will need to support patients by providing guidance and personalised care to meet each patients needs.
