The Hospital at Home service will provide intensive hospital level care for acute conditions that normally require an acute hospital bed, in a patient’s home for a short episode through multidisciplinary healthcare teams. The service will take appropriate patients out of hospital early to help release capacity in the hospital, deliver appropriate care and interventions and prevent unnecessary admission or attendance at hospital. The service is delivered by a team of specially trained registered professionals including GP’s who visit each patient in their home to deliver the care they need.



The post holder will work autonomously and in collaboration with Acute Trusts, the Adult Community Nursing Team, other health care professionals and statutory and voluntary agencies to deliver excellent standards of patient care.

The post holder will be required to:

Work on a rotational shift basis within the Harrow H@H – service operating hours 8 am – 8 pm, 7 days a week.

Prioritise and manage own workload with minimal supervision.
