Summary
Clinical Practice As a trainee you will, under appropriate supervision: Undertake structured clinical assessments, including historytaking, examinations, and development of differential diagnoses. Request and interpret investigations with increasing independence as competency is achieved. Contribute to the development of individualised treatment plans, escalating to senior clinicians when required. Participate in daily Acute Medicine ward rounds, contributing to clinical decisionmaking, documentation, and multidisciplinary discussion. Assessing and clerking patients referred to Acute Medicine, identifying acuity and escalating promptly when concerns arise. Develop skills in recognising and managing deteriorating patients, with support from senior clinicians during emergency situations. Support safe patient flow through highquality documentation, early escalation, and collaborative working. Your level of independence will increase as you meet the competencies outlined in your training pathway. Leadership and Management With guidance and supervision, you will: Contribute to quality improvement projects and audits within the Acute Medicine service and Same Day Emergency Care Develop leadership behaviours through role modelling, reflective practice, and participation in team activities. Contribute to creating a positive learning environment. Participate in governance processes to build understanding of risk, safety, and quality within the department. Education and Training As part of your trainee development, you will: Engage in a structured training plan aligned to your MSc in Advanced Clinical Practice and the requirements of the ACP Framework. Participate in supervised clinical sessions to develop your assessment, diagnostic, and decisionmaking skills. Participate in teaching activities for students and junior staff. Maintain regular meetings with your clinical supervisor and academic mentors to support your progression. Research and EvidenceBased Practice With support, you will: Apply evidencebased practice to your clinical reasoning. Contribute to audits, evaluations, and service improvement projects. Begin to develop skills in reviewing and implementing clinical guidelines and pathways. Professional Portfolio and Governance As a trainee ACP, you will be expected to: Maintain a comprehensive clinical portfolio, demonstrating your progression across the four pillars of Advanced Practice. Work toward meeting the competencies required for ACP qualification. Engage in regular supervision, work-based assessments, and formal review of your academic and clinical development. Adhere to the MultiProfessional Framework for Advanced Clinical Practice and, where applicable, the NMC Principles of Advanced Practice. Communication and Team Working You will: Communicate effectively with patients, carers, and colleagues, seeking support when dealing with complex or sensitive situations. Contribute to handovers and maintain accurate clinical documentation. Develop strong working relationships across Acute Medicine, ED, specialty teams, and community services to support continuity of care.
