Summary

Key Responsibilities Project and Programme Support Support the planning, delivery, and monitoring of PCN projects, including service development and transformation work Maintain project plans, action logs, risk registers, and timelines Track progress against milestones and flag risks or delays to the PCN Manager Support data collection and reporting for PCN programmes and contractual requirements Administration and Coordination Arrange meetings, prepare agendas, take minutes, and follow up actions Coordinate communication between practices, PCN teams, and external partners Collation of invoices and support in the claim and payment process Maintain accurate records, shared folders, and documentation Support submission of reports, returns, and evidence for commissioners Data and Reporting Collect, collate, and present information from multiple sources Support preparation of dashboards, reports, and presentations Ensure data is handled securely and in line with information governance requirements Support delivery of the PCNs digital strategic development Stakeholder Support Act as a point of contact for PCN projects Support engagement with practices, community services, and other partners Provide practical support to clinicians, ARRS roles, and managers involved in PCN work General Support continuous improvement by identifying more efficient ways of working Contribute to a positive, collaborative PCN culture Undertake any other reasonable duties appropriate to the role Responsible for PCN asset register. Undertake any other reasonable duties as required.
