Summary
As our Older Adults Nurse, you will play a vital role in supporting older adults in the community. Key responsibilities include: Care Coordination: Acting as a vital link between care homes and the GP practice. Attending weekly neighbourhood team meetings and carrying out weekly visits to each care home. Patient Assessment & Management: Undertaking comprehensive health assessments for adults aged 65+, supporting chronic condition management, and working proactively to prevent avoidable hospital admissions. Non-Medical Prescribing (if qualified): Prescribing medications and treatments within agreed clinical protocols. Wound Care: Providing specialist care for chronic and complex wounds, including Doppler and ABPI assessments. Immunisations: Administering vaccinations in line with national schedules. Collaboration: Working closely with GPs, care home staff, multidisciplinary teams, and families to ensure holistic care. Additional Duties: Including suture/clip removal, ECGs, venepuncture, and pathology requests where required.
