Oversee the daily operations of the outpatients and wellbeing service, ensuringhigh standardsof care and a positive patient experience. Support service planning, pathway development, and improvements in patient flow. Liaise closely with the Patient Flow Team and clinical staff to ensure safe and efficient operations. Support the inpatient unit during periods of need to ensure continuity of care. Clinical Duties (Within Scope of Practice) Undertake venepuncture, cannulation, and administration of IV therapies safely and competently. Conduct clinical observations, assessments, and monitoring aligned with hospice procedures and within professional boundaries. Undertake holistic assessments contributing to care planning, including physical, emotional, social, and spiritual needs. Support the wellbeing and rehabilitation programme, contributing to group and one-to-one patient interventions. Escalate any clinical concerns promptly to theappropriate medicalor senior nursing staff. Contribute to triage, patient flow management, and safe decisionmaking within the outpatient service. Patient and Family Support Support palliative patients and their loved ones with sensitivity, compassion, and respect. Provide clear, empathetic communicationregardingcare, appointments, and wellbeing support. Participate in advance care planning conversations asappropriate tothe role. Multidisciplinary Teamwork Maintain strong working relationships with the nursing team, medical staff, and wider MDT. Participate in MDT meetings to share information, contribute to planning, and advocate for patient needs. Liaise with external partners including GPs, district nurses, specialist nurses, and hospital teams to support integrated care. Governance, Safety & Quality Improvement Maintainaccurateandtimelyclinical records, including discharge summaries, outpatient notes, and communication with GPs whenappropriate. Identifyand report incidents, risks, or concerns, contributing to a safe clinical environment. Participate in clinical governance activities including audits, evaluation, service reporting, and quality improvement projects. Support the implementation of best practice guidelines and contribute to developing new protocols for the outpatient service. Participate in internal and external presentations. Professional Development Engage in ongoing training, mandatory updates, and clinical supervision. Participate in personal development reviews andmaintainprofessional competencies relevant to the role. Contribute to the strategic development of hospice outpatient and wellbeing services.
