Summary
As a Care Navigator, you will: Be the first point of contact for patients, providing a friendly and professional service. Signpost patients to the appropriate healthcare professional based on their needs. Support administrative duties, including appointment scheduling, handling queries, and processing prescriptions. Work collaboratively within a multidisciplinary team to ensure efficient patient care. Maintain confidentiality and adhere to data protection policies.
