Summary
Working collaboratively and in partnership with other statutory, non-statutory and voluntary teams, agencies and organisations. Nottinghamshire Healthcare Trust currently have a vacancy within the In-Reach department, which sits within the Urgent Care Teams. The In-Reach Team is a small team consisting of a Team Leader, two Band 7 Clinical Leads and four Band 4 Practitioners. This team works across sites at Sherwood Oaks and Highbury Hospital in Bulwell. This role sits directly within the remit of the acute care pathway the post holder will require a good, working knowledge of the functions of the team but will also need to have a sound working knowledge of local resources available in each individual locality (Nottingham City, Bassetlaw, Mansfield & Ashfield, and Newark and Sherwood). You will be required to implement identified tasks and potentially to act as a conduit of communication between health, housing and benefits / social care departments with ease. We currently have 1 positions available, based at Sherwood Oaks. This position is a full-time permanent post. Working hours are Monday to Friday, 9am to 5pm. Responsibilities: To demonstrate high level communication skills.To be proactive in attending the ward to undertake a full assessment of needs on a face-to-face basis.To actively engage the service user with appropriate statutory and non-statutory organisations.To engage with family / carers where appropriate in relation to social difficulties if agreed.To actively attend county-wide DTOC / bed management meetings, utilising problem-solving techniques in order to engage with service users where there are difficulties surrounding discharge. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
