Summary

Prioritise caseload efficiently and effectively and support junior colleagues to do the same. Undertake comprehensive occupation focussed, patient centred assessment of patients, including those with diverse or complex presentations and/or multiple pathologies both in the acute hospital setting and through home visits. Make clinical judgements which require analysis, interpretation and comparison of a range of different interventions and treatment techniques, to optimise physical and cognitive occupational performance capabilities. Formulate and deliver an individualised, patient centred occupational therapy treatment programme using appropriate assessment, clinical reasoning skills and knowledge of treatments and interventions with patients wishes at the centre. To determine patients understanding of each therapy intervention in order to gain valid, informed consent and to be able to work within a legal framework with patients who lack capacity to consent to treatment. To fully engage in learning objectives and development opportunities. The post holder will undertake comprehensive Occupational Therapy assessments of patients by using both standardised and non standardised assessments to identify deficits and skills potentially impacting the patients occupational performance. The timing of the assessment will be driven by clinical need and with an awareness of targets. The post holder will be expected to use this assessment information to formulate treatment plans for patients and allocate treatment sessions as appropriate for both themselves and support staff. These decisions will take into account National standards for Treatment, Trust priorities and Occupational Therapy Department priorities. Treatment plans will take into account patients own goals and those of the multi disciplinary team to predict achievable outcomes and recommend the best course of intervention. It will frequently require advanced communication and problem-solving skills. The post holder will be expected to evaluate treatment plans using outcome measures and make adaptation to the individual treatment programme as indicated. The post holder will have advanced knowledge about health ageing and frailty, incorporating this into assessment and treatment for the acute episode of care as well as supporting the patient with considering factors that can influence extended time in optimum health and quality of life. The post holder will be expected to make clinical judgements involving complex factors or situations, which require the analysis, interpretation and comparison of a range of options.
