About us
The Vale of Glamorgan Long Term Care Service assesses and supports people and their carers with long term needs for care, and support. The team responds to individuals whose needs may be complex or require on-going monitoring and support in order to achieve their wellbeing outcomes. The team supports people where short-term services are not appropriate.

The service also incorporates a dedicated Review team where this post is located.

This team completes scheduled and unscheduled reviews to evaluate progress towards achieving outcomes, establishing that any managed care and support arrangements remain eligible for local authority support, and completing re-assessments where required.

Previous experience of working with Adults is desirable but not essential.

About the role
Pay Details: Grade 8/9 scp 26-30 £37,280-£40,777 Pro Rata £19.32 - £21.14 Per Hour Grade 9 scp 31-35 £41,771-£46,142 Pro Rata £21.65 - £23.92 Per Hour
Hours of Work: 37 hours
Main Place of Work: Canolfan Ty Jenner, Barry

The role include undertaking assessments for care and support , CHC assessments and working with individuals whose needs may be complex or require on-going monitoring and support

About you
You will need:

Experience of working with adults who are vulnerable through illness, disability or age related frailty
Assessment of Adult service users and carers.
Understanding of illness, disability and age related frailty and its effects on individuals/carers/family
Knowledge of relevant legislation eg Social Services and Well-being (Wales) Act 2014, Mental Health Measure (Wales) 2010, Mental Capacity Act 2005 and Mental Health Act 1983.
Knowledge of Safeguarding issues and procedures in relation to Adults
A good standard of general education.
A recognised Degree/Diploma or equivalent qualification in Social Work i.e. Degree, DipSW, CQSW etc.
Registration with Social Care Wales.
