Summary
Our Outreach service operates within a community-based setting, providing personalised support to individuals by enabling and promoting independent living. We focus on empowering individuals to live as independently as possible by offering assistance tailored to their needs and personal goals.
We provide support with essential daily tasks such as shopping, attending appointments, and managing household responsibilities. Our aim is to support individuals with the skills and confidence to navigate their daily routines while fostering self-reliance.
Additionally, we encourage participation in a variety of social and recreational activities like going to the cinema, visiting local cafes, and attending community events. These activities help individuals engage with their community, build meaningful connections, and enjoy a fulfilling and inclusive lifestyle.
Responsibilities of the Support Worker will include:
>Healthcare Support: Assist with healthcare needs, including medication administration and personal care, ensuring the well-being of individuals.
>Encouraging Independence: Promote independence and personal growth by following Support Plans tailored to each individual's needs and goals.
>Record Keeping: Maintain accurate and up-to-date records of daily tasks and activities, ensuring clear documentation of progress and support provided.
>Collaborative Care: Work closely with colleagues, families, and individuals to deliver high-quality care and support, fostering a team-oriented approach.
Operational hours:
Our service operates 7 days a week, offering flexible working hours between 8:00am and 10:00pm. Many of the services start at the individuals homes which span across the Great Glasgow Area.
