Summary
Primary Care Networks PCN are a group of local General Practice surgeries working in collaboration with one another to provide services to its local communities and patient groups. Lakeside Healthcare Group is responsible for 4 PCNs across 3 Localities and providing services to approximately 200,000 patients across these networks. The Personalised Care roles Social Prescribing Link Workers, Health & Wellbeing Coaches and Care Coordinators play an important part in providing extra time, capacity, and expertise to support patients to be actively involved in managing their health and to make choices that are right for them. Working to the What Matters To Me model, personalised care delivered is focused on the strengths and needs of the individual.
