Summary
Clinical: To undertake a comprehensive assessment of patients including those with diverse or complex presentations/multi pathologies; use advanced clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition. Undertake video and telephone assessments as well as face to face sessions. Formulate and deliver an individual therapy treatment programme using assessment, reasoning skills and treatment skills based on a sound knowledge of evidence based practice and treatment options. Formulate accurate diagnosis prognoses and recommend best course of intervention, developing comprehensive discharge plans. To take delegated responsibility from the Band 7 therapist for managing patients with particular conditions and be responsible for providing specialist therapy assessment and treatment plans for patients with these conditions. Evaluate patient progress, reassess and alter treatment programmes as required. Work as a lone practitioner with telephone support from a senior therapist if required. To demonstrate highly developed dexterity, co-ordination, palpatory and other senses for assessment and manual treatment of patients To communicate effectively with patients and carers to maximise rehabilitation potential and to ensure understanding of their condition To assess capacity, gain valid informed consent and have the ability to work within a legal framework with patients who lack capacity to consent to treatment To communicate effectively and work collaboratively with medical, nursing, therapy staff and other health and social care and education professionals to ensure delivery of a coordinated multidisciplinary service. This will include discharged planning and other forms of verbal/written communication as necessary including report writing, referrals to other agencies To communicate highly sensitive information effectively with all patient groups/carers were there maybe difficulties in accepting or understanding information To maintain tact and diplomacy and use highly developed communication skill when working in situations which may become hostile/antagonistic To maintain accurate, comprehensive and up to date documentation in line with legal and departmental requirements, and communicate assessment and treatment results to the appropriate disciplines in the form of reports and letters To undertake as directed the collection of appropriate data and statistics for the use of the department To adhere to the HCPC code of Ethics and Professional Conduct and facilitate compliance of the occupational therapy/physiotherapy staff with local and national standards of practice Work within Trust clinical guidelines and CSP guidelines and to have a good working knowledge of national and local standards and monitor own and others quality practice as appropriate Managerial: To be professional and legally responsible and accountable for all aspects of your own work including the management of patients in your care. To ensure a high standard of clinical care for the patient under your management, and support more junior staff to do likewise Implement policy and service development changes as directed To take responsibility for Health and Safety aspects of your work and implement any policies that maybe required to improve the safety of your work area, including your prompt recording and reporting of accidents to senior staff, and ensuring equipment use is safe To demonstrate a sound understanding of clinical governance and risk management and apply to work situation To undertake the measurements and evaluation of your work and current practices through the use of evidence based practice projects, audit and outcome measures under the guidance of the Senior or Lead Clinician.
