Summary
Enable patients to maintain and improve their independence by promoting health and wellbeing. Undertake physical health checks and physical health care as directed by the Registered Nurse and service specification, reporting outcomes on the Primary Care electronic system. Report physical health concerns identified during the checks to the GP or practice nurse.
