Summary
We are looking for a compassionate and motivated Health Care Support Worker (Band 3) to join our Community Nursing and Rehabilitation teams. This is a rewarding opportunity to make a real difference to patients’ lives by supporting them to remain independent and receive high quality care in their own homes and community settings.
Working under the guidance and supervision of a registered nurse or allied health professional, you will provide a range of general and specialist care tailored to individual patient needs. You will be supported to develop your skills and confidence through structured training and competency assessments, enabling you to grow within your role and deliver care safely and effectively.
As a valued member of the team, you will play an important role in delivering person centred care, supporting service performance, and maintaining high standards of quality and compassion across the service.
• Deliver high quality, person centred care to patients under the supervision of registered nurses and allied health professionals
• Undertake delegated clinical tasks in line with training and competency, including observations, glucose monitoring, urinalysis, mobility support and promoting independence with activities of daily living
• Contribute to patient assessment by observing, recording and reporting clinical and behavioural information accurately and promptly
• Recognise and escalate any changes in a patient’s condition to the appropriate clinician in a timely manner
• Support patients and carers by providing information, reassurance and guidance to help them make effective use of services
• Communicate effectively with patients, carers and the multidisciplinary team, adapting communication to individual needs
• Maintain a safe, clean and compliant working environment in line with infection prevention, health and safety and lone working policies
• Contribute to service improvement, audits and quality initiatives as required
• Maintain mandatory training, competencies and an up to date CPD portfolio
• Support the induction, training and development of new staff and students where appropriate
