Summary
Deliver high-quality clinical care to residents in care homes in line with the EHCH DES, including regular scheduled care home rounds. Undertake comprehensive clinical assessments, diagnosis, and management of patients with frailty, multi-morbidity, and long-term conditions. Provide planned and urgent community home visits to support early intervention and prevent unnecessary hospital admissions. Prescribe independently within scope of practice (V300), escalating complex or high-risk cases to the ACP Lead or GP as appropriate. Contribute to MDT meetings, case reviews, and care planning with PCN, community, and care home colleagues. Support personalised care planning, including Advance Care Planning, Treatment Escalation Plans, and end-of-life care. Maintain accurate and timely clinical records in line with professional and organisational standards. Participate in clinical governance, audit, and quality improvement activities aligned to EHCH and frailty pathways. Post holder must be available to work weekends on a rota basis and be committed to working across multi sites and within community completing home visits and regular visits to our aligned care homes.
