Summary

Role: Pharmacist Salary: Aligned to experience, qualification, and agreed workload Hours: Up to full time (37.5hours) Location: Debenham/Otley/Grundisburgh Job Summary We are seeking a Pharmacist following the planned departure of our current postholder. The role is embedded within the day-to-day clinical activity of the practice and focuses on direct patient care, medicines optimisation, and prescribing safety. You will work closely with GPs, nursing staff, our dispensary and the wider practice team, delivering structured medication reviews, running patient-facing clinics (including askmyGP-allocated work), and contributing to audit and quality improvement activity. The role is well suited to an experienced pharmacist with independent prescribing status, or a pharmacist willing to work towards this through the PCN-supported CPPE training pathway where appropriate. The post is clinically focused, with scope to shape and develop medicines-related systems and processes within the practice. Key Responsibilities Clinical Responsibilities Undertake structured medication reviews, including polypharmacy and deprescribing work, with particular focus on: - patients with frailty - multiple long-term conditions - high-risk medicines Run patient clinics, including work allocated via askmyGP, addressing medicines-related queries, optimisation, and follow-up. Manage medicines for patients with long-term conditions within your competence (e.g. hypertension, asthma, COPD, Type 2 diabetes), in line with local protocols and prescribing authority. Provide medicines reconciliation following hospital discharge and other transfers of care. Conduct care home and frailty medication reviews, where appropriate. Act as a point of contact for patients regarding medicines-related queries and concerns. Prescribing and Medicines Optimisation Prescribe independently (where qualified) in accordance with national guidance, local protocols, and practice policy. Support safer prescribing systems, including monitoring requirements and follow-up. Contribute to antimicrobial stewardship and medicines safety initiatives. Action and support implementation of MHRA safety alerts and other national prescribing guidance. Support the practice in achieving QOF medicines-related indicators through optimisation of prescribing and disease registers. Audit, Quality Improvement, and Systems Lead or contribute to prescribing safety audits and clinical audit activity. Support the development and maintenance of prescription management systems and processes, working alongside clinical and administrative staff. Identify opportunities for quality improvement in medicines use and patient outcomes and support implementation of agreed changes. Contribute to reducing medicines-related workload for GPs through safe delegation and system improvement. Professional Responsibilities Maintain registration with the General Pharmaceutical Council (GPhC) and comply with professional codes of conduct. Maintain and develop professional competence through continuing professional development. Participate in appraisal, agree objectives, and maintain a personal development plan. Where appropriate, undertake or work towards independent prescribing qualification through the CPPE pathway, with appropriate supervision and support. Attend relevant meetings and training as agreed with the practice. General Work flexibly across Debenham, Otley, and Grundisburgh sites as required. Plan and organise your own workload effectively, balancing clinical activity, audit, and development work. Undertake any other duties commensurate with the role, as agreed with the line manager. Additional Notes Independent Prescriber status is highly desirable, but applications from experienced pharmacists willing to work towards this will be considered. We offer a very supportive environment with established systems and close multidisciplinary working. Some remote working may be supported following successful completion of probationary period.
