Summary
This role requires confidence, effective communication, diplomacy, and the ability to provide constructive feedback while working in complex and demanding environments. The post holder will complete commissioner oversight visits, ensuring individuals with LD/A who may be detained receive safe and appropriate care and have clear discharge plans. Working closely with system partners and care providers, the role focuses on identifying risk, improving services, and strengthening governance MainDuties and Responsibilities Coordinate and deliver Care (Education) and Treatment Reviews for the Assuring Transformation cohort in line with NHSE timeframes. Participate in regular reviews of the Dynamic Support Register with system partners to ensure accurate identification of individuals at risk of admission. Apply and chair the Local Emergency Area Protocol (LEAP) for individuals at imminent risk of inpatient admission, supporting community services throughout the process. Work collaboratively with local authorities and system partners to enable timely, personcentred discharges. Complete quality assurance visits for all individuals in BNSSGs LD/A cohort, following Commissioner Oversight guidance. Support achievement of NHSE discharge targets for the BNSSG area. Maintain positive working relationships with a wide range of internal and external stakeholders. Participate in internal and external working groups, providing analytical input and specialist advice. Present complex information clearly to varied stakeholder groups. Lead agreed areas of performance improvement and contribute to strategic planning, ensuring inter-dependencies and resource needs are identified. Support the development of policies and procedures impacting the wider organisation. Provide support to the wider Funded Care Team as required.
