Summary
To undertake comprehensive assessments of service users physical, psychological, social and emotional needs, and to develop, implement and regularly review individualised care plans. To lead the clinical management of a designated group of service users, coordinating care delivery and prioritising clinical needs to ensure safe and effective outcomes. To coordinate the clinical environment on a shift-by-shift basis, ensuring appropriate staffing, delegation of duties and continuity of care. To observe, report and accurately document changes in service users physical condition, mental state and behaviour, escalating concerns appropriately. To work collaboratively with the MDT, including medical staff, allied health professionals, social care and external agencies, contributing to care planning and review. To maintain up-to-date knowledge of relevant mental health and learning disability legislation, national guidance, Trust policies and evidence-based practice, applying this to clinical decision-making. To identify, provide and supervise the delivery of physical care to support activities of daily living, including personal hygiene, nutrition and hydration, mobility, manual handling, elimination, sleep and rest, and maintaining a safe personal environment. To undertake and supervise prescribed physical health observations, including blood pressure, temperature, respirations, blood glucose monitoring, urinalysis, weight and height, reporting and acting on abnormal findings. To ensure the safe and accurate administration of prescribed medication in line with Trust policy, and to monitor and document therapeutic effects and adverse reactions. To provide therapeutic interventions, emotional support and stability to service users, promoting engagement, recovery and independence while maintaining appropriate professional boundaries. To act in a manner that respects and promotes service users individuality, cultural background, values, sexuality and spiritual beliefs, challenging and addressing any behaviour that undermines equality and diversity. To supervise, mentor and support junior staff and students, facilitating learning opportunities and ensuring they are actively engaged in patient care and therapeutic activities. To create and maintain effective professional relationships with colleagues, other professionals and external agencies to support integrated care and positive service user outcomes. To work within Trust and professional guidelines relating to consent, capacity and safeguarding, ensuring service users rights and best interests are upheld. To demonstrate an understanding of the range of therapeutic interventions available, the supporting evidence base and potential outcomes for service users, applying this knowledge to practice.
