Summary
The following are the core responsibilities of the Clinician Pharmacist. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Primary Duties Long-term condition clinics Clinical medication review Care home medication reviews Patient facing domiciliary clinical medication review Patient facing medicines support Telephone medicines support Medicine information to practice staff and patients Provide follow up for patients to monitor the effect of any changes. Unplanned hospital admissions Management of medicines at discharge from hospital Supporting cost effective medicines use Repeat prescribing Risk stratification Service development Medicines quality improvement Medicines safety Education and training Care Quality Commission
