Summary

The following are the core responsibilities of the EHUB clinical pharmacist. There may be on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels: Clinical Responsibilities: a. Practice as an independent prescriber working within scope of practice b. Manage online consultation requests routed to pharmacy through the EHUB c. Perform telephone consultations within scope of practice d. Undertake structured and problem focused medication reviews e. Manage medication related requests for long term conditions within scope of practice f. Support medicines optimisation and cost effective prescribing g. Support contractual working relating to chronic conditions and medication management h. Identify and escalate complex or high-risk cases to GPs or other clinicians when required i. To provide advice and support for patients with self-limiting conditions j. To reconcile medicines following patients discharge from hospital or clinic appointments k. Manage shared care medication requests l. Engage with drug monitoring, blood test requests and review m. Synchronise repeat prescriptions where necessary n. Link prescriptions to medical conditions o. Review medications for newly registered patients p. To provide subject matter expertise on medication for broader clinical team q. To reauthorise medication for repeat prescribing within scope of practice r. Contribute to the organisation and running of public health campaigns s. Communicate effectively and regularly with broader team EHUB and Systems Working t. Work across 5 GP practice sites via the shared EHUB Model u. Use PATCHES online consultation platform, managing queries within 48 hours of receipt v. Use Systm1 and associated digital systems (AccuRX / Valida) to record accurate clinical documentation, safety net and follow up plans w. Triage to other clinicians on site for face to face review where necessary x. Work collaboratively with administrators, GPs, nurses and pharmacy colleagues to ensure safe, efficient and timely care y. Make suggestion and contribute to quality improvement in EHUB operations and service development Training, Supervision and Management z. Complete induction and system training across 5 GP Sites aa. Build strong working relationships with GP practice teams and colleagues bb. Undertake onward training to build skills to further ability to support service delivery cc. Support audit led by EHUB pharmacy lead when required dd. Participate in EHUB related governance projects and research ee. Engage in clinical supervision, appraisal and professional development led by EHUB pharmacy lead ff. Attend EHUB team meetings with GPs, nursing, pharmacy In addition to the primary responsibilities, the clinical pharmacist is requested to: a. Keep up to date with latest guidance, supporting practices to conform to NICE, CQC and other relevant standards. b. Act as the practice point of contact for all medicines related matters, establishing positive working relationships within the EHUB system Continuous Professional Development Take responsibility for own personal development and learning, maintaining a personal portfolio of evidence related to practice, keeping up to date with medical and therapeutic evidence and opinion, legislation and policy developments. Proactively keep up to date with and retain, share and grow knowledge base through regular CPD development
