Job Description

As a Home Care Worker at Nurtured Prime Health, you will provide essential care and support to clients in their own homes, helping them to live safely, independently, and with dignity. You will deliver person centred care while maintaining high professional standards.

Key Responsibilities

Provide personal care and support with daily living activities including bathing, grooming, dressing, and toileting.

Support clients with mobility and transfers safely.

Administer medication in line with care plans where trained and authorised.

Prepare and serve meals, ensuring dietary needs and preferences are followed.

Provide companionship and emotional support to promote wellbeing.

Assist with light household tasks such as cleaning and laundry.

Maintain accurate and timely care records.

Monitor and report any changes in a client’s condition or needs to the appropriate supervisor.

Encourage independence and participation in activities that enhance quality of life.

Maintain a safe, clean, and supportive home environment.

Follow all company policies, procedures, and health and safety requirements.

Attend training, supervision, and team meetings as required.

Provide appropriate support and reassurance to clients’ family members when needed.

Qualifications and Requirements

A high school diploma or equivalent. Relevant care qualifications are an advantage.

Previous experience in home care or a similar role is desirable but not essential.

A compassionate, patient, and respectful approach to care.

Good communication and interpersonal skills.

Ability to work independently and as part of a team.

Reliable, punctual, and professional.

Willingness to undergo background and reference checks.

A valid driving licence is mandatory.

Flexibility to work various shifts including evenings, weekends, and holidays.
