Summary

Your role would be to: Proactively identify (using population data) patients with long-term conditions and support their personalised care needs e.g. preparing for a shared decision-making conversation or providing holistic support; Help patients manage their needs through answering queries, making and managing appointments, and ensuring that good quality written or verbal information is available to help with choices about care. Assist with patient queries, signposting, facilitating access to self-management education and support for patients to take more control of their health/wellbeing. Provide coordination and navigation for people and their carers across health and care services, working closely other primary care professionals; and Promoting and supporting PCN health initiatives such as immunisations, health checks, smears Proactively support call and recall process Working proactively with the PCN pharmacist to support medication reviews Excellent interpersonal skills and the ability to interact well with different kinds of people Assisting with the Appointment Book Provide administrative support to the practice. Working with the team to ensure that all clinical rooms are stocked and overseeing the ordering of items needed. A full copy of the job description is attached.
