Summary
The mainresponsibilities are contained within the job description in more detail Internal Relationships: To communicate effectively, with Patients, carers and other staff imparting complex and sensitive information and unwelcome news. Senior physiotherapy staff to maintain a coordinated approach to service delivery and development. With qualified physiotherapy staff, physiotherapy assistants and students on clinical placement to support team working on a day to day basis With medical, nursing, paramedical, and other hospital staff to promote a co-ordinated approach to patient care and multi-disciplinary team working in the preparation and implementation of treatment/care policies, procedure and care pathways. Senior Managers Other Trust departments External Relationships With all disciplines in the community, particularly other physiotherapists, to provide continuity of care across community settings and other acute Trusts To contribute to and execute the Patient Discharge Care Plan through effective communication with the patient, family and carers.
