Crynodeb
Key Responsibilities Assess, diagnose, plan, and evaluate care for housebound patients with undifferentiated, acute, and chronic conditions. Conduct comprehensive assessments, including medical histories, physical examinations, considering physiological, and social factors. Manage complex, urgent, and emergency presentations, initiating appropriate emergency care when required. Proactively identify, diagnose, and manage long-term conditions, including completion of annual QoF reviews and locally commissioned service requirements. Prescribe, review, and optimise medicines within scope of practice and in line with national and local protocols. Deliver clinical interventions including wound care, phlebotomy, and vaccination and immunisation programmes. Develop and implement individualised management plans, referring to or seeking advice from GPs and clinical leads as appropriate. Provide patient education, health promotion, and preventative care to support self-management and long-term wellbeing. Work collaboratively with GPs, nurses, and the wider multidisciplinary team to ensure coordinated, holistic care across home and clinical settings. Maintain accurate and timely clinical documentation on practice systems (e.g. EMIS) and contribute to reducing hospital admissions and improving patient outcomes.
