Summary
Role involves ongoing physiotherapy assessment and rehabilitation of stroke survivors following completion of their hyperacute care. This will commonly include: Hands on physiotherapy therapy and therapeutic handling Leading or supporting group exercise sessions Prescribing of exercise programmes to complete under supervision of other staff / carers / independently Use of treatment adjuncts such as SAEBOMAS, mirror box, therabike, FES - previous experience of these is desirable but not essential as training can be provided Joint treatment sessions with other disciplines Therapeutic involvement in patient ADL's which may include supporting continence needs Goal setting using GAS format - training can be provided Providing advice and education to patient, family and other members of the MDT Family meetings and discharge planning which may occasionally include home visits MDT discussions Supervision and education of other members of the team including band 5's, assistant staff and students. Use of outcome measures as indicated Data collection for SSNAP Liaison with community services and completing ongoing referrals as required.
