Summary
Carrying out risk assessments and formulating appropriate risk management plans Formulating care plans and carrying out treatment interventions as appropriate Safe prescribing and recording of drug information. Physical health monitoring of patients including carrying out physical examination and investigations as appropriate. Post holder will be required to do necessary documentation in the patients electronic notes and liaise with the ward multidisciplinary team and the Consultant as well as other services including carers and relatives, to optimise care. You may be required to make referrals to other services and complete discharge and other relevant documentation in a timely manner. Liaising with GPs and various other professionals/ teams within and outside SLAM including with social services, DVLA and the Voluntary sector. For further details please refer to attached JD.
