Summary
Key ResponsibilitiesPatient Care and Coordination Act as a first point of contact for patients, responding to queries via telephone, email, text, and letter. Support care navigation by directing patients to appropriate services and professionals. Liaise with internal and external stakeholders to ensure seamless and integrated patient care. Assist with Flu and Covid vaccination clinics as required. Develop the role to include phlebotomy and basic Healthcare Assistant duties (subject to training and competency). Administrative Duties Manage document workflow, prescriptions, tasks, and practice email inboxes. Provide administrative support to the Patient Participation Group, including organising agendas, taking minutes, and managing associated paperwork. Support practice projects and initiatives such as Care Navigation and patient engagement activities. Contribute to the practice newsletter and update waiting room media displays. Use clinical IT systems (e.g., SystmOne) to record and manage patient information accurately. Team Working Work collaboratively with GPs, nurses, allied health professionals, and administrative staff. Support continuity of care and integrated working within the Primary Care Network and partner organisations. Maintain confidentiality and comply with practice policies and NHS governance standards.
