Summary
Partnership Working & System Leadership Act as a senior clinical leader for partnership working across the ICS, Place, PrimaryCare Networks, local authorities, VCSE organisations and other system partners. Strengthen and support collaborative working with Primary Care and key VoluntarySector partners. Work closely with the Director of Partnerships and Transformation to ensure that theTrust is represent in relevant ICS, place-based and multi-agency forums relating topopulation health, prevention and inequalities, with a focus on Person-centred care. Strengthen clinical engagement in system redesign initiatives that shift care closer tohome and improve integration between hospital and community services.
