Summary
The Clinical Specialist Nurse will take a lead role in delivery of clinical service to the patients, families and paid/unpaid carers we support. This will include supervision of clinicians and playing an active role in collective clinical decision-making. The post holder will have responsibility for providing clinical leadership to colleagues in assessment, treatment and management of patients with advanced Dementia and complex, multifaceted needs. The post holder will contribute to the planning, oversight and provision of expert care and support to all patients who are transferred to the ward, providing evidenced based interventions based on need and promoting the soonest possible safe transfer into the community. The post holder will also formulate plans of care, positive behaviour support and transfer plans with the multi-disciplinary team and negotiate implementations of such plans and the sharing of complex, sensitive and confidential information. The post holder will facilitate and support the development and delivery of innovative dementia training and education to staff, to improve competence and confidence, in line with best practice and guidance. They will work with the PDP and Ward Manager to ensure learning from safety events and audits, developing realistic, timely and impactful improvement action plans.
