Summary
To administer an assessment tool for ADHD, QB Test, following training. To check and maintain a safe environment. To be familiar with all equipment required to support the child and to report any deficiency immediately. To complete physical monitoring of young people, such as height, weight, pulse and blood pressure and record these accurately. To identify any concerns about a child or young persons physical health and report to the multi-disciplinary team. To regularly liaise with the multi-disciplinary team and present complex and sensitive information in a clear way. To work as a member of the multi-disciplinary team and with other professionals or representatives of voluntary organisations who also work with young people with ADHD, ASD and their families. To communicate effectively with colleagues and other members of the team and identify and report concerns to senior staff. To communicate effectively with families, carers and young people. To make verbal and written reports as required, ensuring these are kept up-to-date. To keep accurate and clear records. To maintain timely and up to date reports in the family held record.
