Summary
Clinical Provide timely and effective assessment and triaging of patients presenting with sickle cell crisis,supporting and caring for patients whilst balancing their requirements with the Trust serviceprovision and resources. Provide expert clinical advice, support and guidance for patients, carers and allied healthcareprofessionals involved in providing care for patients with sickle cell disease. Offer timely and appropriate clinical decision-making and interventions where appropriate andescalate as required. Work closely with haematologists, bed managers, ward managers and nurses to initiate timelyadmission and discharge planning. Facilitate nurse led discharges. Act as a role model and clinical resource, demonstrating high standards of care and providingclinical leadership in the specialist area Promote good clinical practice in line with evidence-based guidelines and policies To liaise with relevant A&E staff and other members of the MDT to optimize a smoothtreatment pathway for patients in ambulatory care triage. Develop and run nurse led clinics To contribute to the education of patients about their condition, enabling them to makeinformed choices
