Neway International are seeking a Social Worker to join our client based with Bromley Council’s Preparing for Adulthood (PfA) Team.
Location: Churchill Court, 2 Westmoreland Road, Bromley, Kent, BR1 1AS
Hours: 36 per week
Start Date: 02 February 2026
Work Pattern: Hybrid – 2 days office, 3 days WFH
About the Client
Bromley Council’s Preparing for Adulthood (PfA) Team sits within the 0–25 Service, supporting children and young people with disabilities as they transition into adulthood. The team focuses on promoting independence, education, employment, community inclusion, and positive wellbeing outcomes.
The Role
The Social Worker will support young people aged 14–25, working jointly with Children’s Social Care from 14–18 and taking full case responsibility from 18 onwards. The role involves completing Care Act assessments, assessing capacity, developing person‑centred care packages, and supporting families through the transition into adult services. This is a fast‑paced locum role requiring immediate availability.
Key Responsibilities
Manage a caseload of young people aged 14–25 within the PfA pathway.
Complete Care Act assessments, capacity assessments, and person‑centred support plans.
Work jointly with Children’s Social Care and take full responsibility for cases from age 18.
Support transitions into adult care services and ensure continuity of support.
Work closely with families, education providers, health professionals, and partner agencies.
Maintain accurate records and ensure compliance with statutory requirements.
Promote independence, wellbeing, and positive outcomes for young people with disabilities.
Candidate Requirements
Qualified Social Worker with Social Work England registration.
Experience working with young people with disabilities and/or within 0–25 or transitions services.
Strong understanding of the Care Act, capacity assessments, and adult social care pathways.
Ability to work collaboratively with multi‑agency partners.
Strong communication, assessment, and report‑writing skills.
Must be immediately available (start within 1–2 weeks).
