Summary

We're looking to recruit a Housing First Practitioner in the Halton region to join a specialist homelessness and housing support service. This is a full‑time role working 35 hours per week on a rotating shift pattern between 08:30-20:30, including 1 weekend in every 8.

You'll provide wrap‑around, flexible, and open‑ended support to people who have experienced homelessness or rough sleeping, helping them to sustain tenancies, access community services, and work towards personal goals. The role involves a blend of outreach, home visits, office‑based work, and home working.


Key responsibilities:

  • Build trusting, professional relationships with people experiencing homelessness or insecure accommodation.
  • Manage a small caseload using Housing First principles.
  • Deliver intensive, strengths‑based and person‑centred support.
  • Complete support plans, risk assessments and safety plans, reviewing progress regularly.
  • Provide practical tenancy sustainment support (benefits, budgeting, appointments, accommodation).
  • Identify and manage risks through dynamic risk assessment.
  • Work collaboratively with landlords, health services, support agencies and community partners.
  • Encourage service‑user involvement in shaping their support.

What you'll need to succeed

  • Experience supporting vulnerable adults facing homelessness, rough sleeping or multiple complex needs.
  • Knowledge of substance use, mental health, welfare benefits, housing pathways or homelessness law.
  • Strong understanding of trauma‑informed and strengths‑based practice.
  • Confident in assessments, support planning, risk management and safeguarding.
  • Ability to manage challenging behaviour with empathy and professional boundaries.
  • Experience helping individuals sustain tenancies and engage with services.
  • Confident lone working, organising a caseload and working in the community.
  • Strong partnership‑working skills with multi‑agency partners.
