Summary
Clinical Responsibilities Independently assess, diagnose, and manage patients presenting with minor illness Undertake comprehensive clinical assessments including history taking, physical examination, diagnosis, treatment, and referral as appropriate Prescribe safely and effectively in line with current legislation and local guidelines Request, interpret, and act upon investigations Refer patients to secondary care or other services when clinically indicated Maintain accurate and contemporaneous clinical records Long-Term Conditions & QOF Support the management and review of patients with long-term conditions (e.g. asthma, COPD, diabetes, hypertension) Contribute to achieving QOF targets through high-quality, evidence-based care Participate in chronic disease clinics as required Work collaboratively with the practice team to improve patient outcomes Professional & Team Working Work closely with GPs, nurses, HCAs, pharmacists, and the wider MDT Participate in clinical meetings, audits, and service development Maintain professional registration and engage in ongoing CPD Adhere to clinical governance, safeguarding, and infection control policies
