Summary
Act as a senior clinical decision maker within the Hospital @Home service Assess, diagnose and manage acutely unwell patients in their own homes Develop and review care plans, support safe discharge and prevent hospital admission Provide clinical leadership, supervision and advice to the multidisciplinary team Participate in MDT meetings, handovers and service governance activities Manage investigations, referrals, escalation of care and safeguarding concerns Contribute to quality improvement, audit, education and service development Maintain professional standards including appraisal, revalidation and CPD Work flexibly across community sites, including weekend working as required *** Please be advised that this vacancy may close early should we receive a sufficient number of applications ***
