Summary
Key Responsibilities: Clinical Care Provide person-centred care and support patients with a wide range of health needs. Perform and document clinical observations including blood pressure, temperature, pulse and respirations. Carry out delegated annual reviews and routine assessments under supervision. Deliver vaccination and immunisation programmes including flu and childhood vaccines once trained. Undertake ECGs, venipuncture and other clinical procedures within the scope of competence. Assist registered nurses and GPs with minor surgery, family planning clinics and treatment sessions. Provide general health advice, lifestyle guidance and opportunistic health promotion. Offer information and support related to contraception and family planning. Assess and manage uncomplicated wounds within practice protocols. Support patients with dementia, mental-health needs and learning disabilities. Recognise clinical deterioration and escalate concerns promptly to senior staff.Safety, Safeguarding and Emergency Response Recognise signs of safeguarding concerns involving children, adults or vulnerable individuals, following local and national safeguarding procedures. Identify, respond to and manage emergencies such as anaphylaxis and cardiac arrest, following current UK guidelines. Maintain infection-prevention and control standards, ensuring safe clinical practice at all times. Ensure correct storage, handling and monitoring of clinical equipment, vaccines and emergency supplies.Communication Communicate sensitively and effectively with patients, carers and MDT members. Support patients in understanding treatment plans, medications and clinical instructions. Use alternative communication methods where necessary to meet individual patient needs. Maintain confidentiality of all patient, staff and organisational information.Quality, Governance and Service Improvement Support practice audits, QOF activity, CQC compliance and data-collection requirements. Contribute to quality improvement projects and local incentive schemes relevant to the role. Produce accurate, timely and compliant documentation in accordance with NMC standards. Assist with service evaluation, reporting and monitoring of care quality. Work in partnership with clinical teams to improve patient experience and service outcomes. Teamwork and Collaboration Work as an effective member of the multidisciplinary team, supporting nurses, GPs, HCAs and clinical support staff. Follow appropriate delegation pathways and seek senior support when needed. Build and maintain positive working relationships across the MDT and wider PCN. Support integration of nursing associate duties across primary, community and mental-health services. Assist colleagues with workload prioritisation and contribute to smooth daily service delivery.Professionalism and Development Maintain professional boundaries and work within the NMC Code and Nursing Associate scope of practice. Complete mandatory training including safeguarding, equality and diversity, confidentiality, and CPR. Engage in reflective practice and maintain a professional development portfolio. Participate in annual appraisal and take responsibility for ongoing learning. Attend additional training to develop enhanced clinical skills as required by the practice.General Responsibilities Preserve confidentiality of patient and organisational information at all times.
