Adult Community Team –Inspiring Independence, Nurturing Wellbeing
Are you passionate about helping people live independently and achieve their goals? As a Social Care Practitioner, you will be at the heart of improving lives and wellbeing across Wiltshire. You will join a forward-thinking team that values creativity, collaboration, and professional growth - where your ideas truly matter.
In this rewarding role, you will manage your own caseload and work closely with customers, carers, and partner agencies to deliver person-centred care. Working collaboratively with Occupational Therapists, Social Workers, and multi-professional teams, you will promote independence, choice, and wellbeing using universal services and community resources.
As this is a fast-paced team, you will lean on your organisational ability to manage caseloads effectively and have excellent communication skills to be able to work collaboratively. In addition to a sound understanding of the Care Act 2014, you will hold an NVQ Level 4 in Health and Social Care or equivalent or the ability to work towards this qualification and have experience of working within a Health or Social Care setting.
You will be providing outcome-focused assessments to identify the right support for positive life changes and creating and reviewing support plans that make a real difference.
