Are you passionate about helping people live independently and achieve their goals? As a Social Care Practitioner, you will be at the heart of improving lives and wellbeing across Wiltshire. In this rewarding role, you will manage your own caseload and work closely with customers, carers, and partner agencies to deliver person-centred care. Working collaboratively with Occupational Therapists, Social Workers, and multi-professional teams, you will promote independence, choice, and wellbeing using universal services and community resources.

As this is a fast-paced team, you will lean on your organisational ability to manage caseloads effectively and have excellent communication skills to be able to work collaboratively. In addition to a sound understanding of the Care Act 2014, you will hold an NVQ Level 4 in Health and Social Care or equivalent or the ability to work towards this qualification and have experience of working within a Health or Social Care setting.

You will be providing outcome-focused assessments to identify the right support for positive life changes and creating and reviewing support plans that make a real difference.
