About the Client
Wrexham County Borough Council’s Leaving Care Team provides statutory support to young people transitioning from care into independent adulthood. The service ensures compliance with the Children Act 1989, Children (Leaving Care) Act 2000, and the Social Services and Wellbeing Act 2014, delivering high‑quality, person‑centred support to improve life outcomes for care leavers.

The Role
As a Personal Advisor, you will provide direct support to young people aged 15.9 and above who are preparing to leave care or have already transitioned to independence. You will work closely with social workers, partner agencies, education providers, and housing services to ensure each young person receives the guidance, advocacy, and practical support they need.

This role is essential in helping young people build confidence, develop life skills, and access the right services at the right time.

Key Responsibilities
Act as the allocated Personal Advisor for eligible young people, building positive and professional relationships.

Support the completion and review of Pathway Plans, ensuring statutory requirements are met.

Provide advice on benefits, education, employment, and training opportunities.

Case manage young people aged 18+, reviewing plans and delivering tailored support.

Assist young people in developing independent living skills, including budgeting, tenancy setup, and accessing grants.

Liaise with social workers, colleges, health professionals, housing providers, and partner agencies.

Advocate for young people and ensure safeguarding procedures are followed.

Participate in team meetings, reviews, and multi‑agency discussions.

Maintain accurate records and ensure data is managed in line with confidentiality and data protection policies.

Support young people in crisis situations and respond flexibly to their needs.

Candidate Requirements
Essential:
Minimum of 5 GCSEs (Grade C or above) including Maths and English, or equivalent experience.

NVQ Level 3 in Health & Social Care (or equivalent).

Experience supporting young people with complex needs.

Strong knowledge of the Children Act 1989 and Children (Leaving Care) Act 2000.

Excellent communication skills, both written and verbal.

Strong IT skills.

Ability to work independently and as part of a team.

Ability to manage workload, meet deadlines, and work to statutory timescales.

Ability to travel across the County Borough.

Desirable:
Ability to communicate in Welsh.

Understanding of Welsh language and culture.
