Summary


To work closely with ward staff involved in discharge planning to ensure that the assessments and referrals for services relating to the patients discharge is completed comprehensively and in a timely manner. This will involve patient facing contact.

The post holder will undertake the role of identifying complex discharges and facilitating assessments for the complex discharge of patients to community services which will ensure their safe discharge from acute care.
• To work closely with ward staff involved in discharge planning to ensure that the assessments and referrals for services relating to the patient’s discharge are completed comprehensively and in a timely manner.
• To ensure the delivery and maintenance of a high standard of care that is patient centred and evidence based.
• To work with teams and specialties across the system to facilitate safe discharges in a timely manner.
• Adhere to the NMC Code of Professional Conduct, Guidelines and Standards and ensure colleagues do too.
• Provide high standards of evidence-based patient centred care and knowledge for all patients, respecting dignity and privacy.
• Deliver high quality care for patients within the discharge process and ensure all discharge plans are robust to provide a seamless and cohesive transition to other settings.
• Attend mandatory training as a minimum.
