Summary
Job Responsibilities: To appraise the CYPMHS Nurse Specialist and MDT of the young persons progress and observations of changes in their mental health condition and ensure these changes are documented on the electronic record. To have a close working relationship with the CYPMHS Nurse Specialist and nominated care coordinators. To regularly meet with the supervisor and manager to ensure the post holder is supported to carry out the demands of the post To ensure that all care services provided are offered without prejudice regardless of age, creed, gender and ethnicity To be aware of diverse cultural and ethnic needs and facilitate appropriate support and care. To support clients with activities of daily living, support to attend school and appointments. To accompany the client getting to outpatient, GP and other appointments relevant to current health or social needs as part of the individuals care plan if agreed to do so.
