The post is based in the Neuro Rehab Service. The Team work across in patient, community (Integrated Community Stroke and Neuro Rehab Service) and out-patient settings to provide specialist, evidence based Neuro Rehab to patients following stroke or other Neurological diagnosis. This post is based on the inpatient stroke and neuro ward at North Devon District Hospital. The in-patient team are an integrated physiotherapy and occupational therapy team working on a 24 bedded ward at North Devon Hospital (Staples Ward). They work closely with the broader MDT including the Nursing staff, Doctors, Speech and Language Therapists, Dieticians and Psychologists. The team is specialist in providing acute therapy post stroke and neurological diagnosis. They assess patients physical and cognitive abilities as well as functional ability with things like walking, washing and dressing and activities in the kitchen. They consider the patients previous abilities, home and lifestyle to plan goals and formulate a treatment plan. The therapists, alongside the support worker team will support intensive rehab which includes activity in the ward therapy gym, utilising specialist rehab equipment and facilitating rehab groups and to support patients recovery. The team work closely with patients and families to support them with what has happened and plan for the future. This includes linking with the community teams to ensure a seamless, well planned and well communicated patient focussed discharge. You will also be supported to acquire knowledge and skill which will give you a sound core competency in Stroke care and Neuro Rehab. We are a friendly and dedicated team.
