Summary
Provide and maintain a high standard of skilled, holistic nursing care for patients in their homes, or a suitable alternative setting, using an agreed model of nursing care and employing appropriate additional professional skills e.g. active nurse prescribing. Assess, develop, implement and evaluate programmes of care to meet the holistic needs of individuals, families and priority groups identified within the practice population. Participate in recognised health promotion/surveillance programmes, and be pro-active in instigating health improvement measures appropriate to NSFs and the GMS contracts. Actively raise patients health issues through communication with the Primary Health Care Team and other agencies, either at formal meetings of these organisations, or with representatives on an individual basis. To identify and report any case of suspected or confirmed abuse to an individual in accordance with the agreed guidance and procedure. To be personally accountable for professional and ethical actions and omissions and ensure compliance with NMC Code of Conduct. Use professional knowledge and judgements to take into account the range of care package and treatment options available. Give full consideration to the patients personal circumstances and preferences in order to formulate and adjust programmes of care. Execute tasks delegated by the Operational Lead concerning the day to day organisation and operation of the nursing team, for example off duty rotas. Assume responsibility for the supervision and co-ordination of the whole caseload and nursing team in the absence of the Operational Lead. Maintain accurate records as required by the Health Authority and in accordance with NMC guidelines. Correlate statistical information as required by team management. Participate in clinical audit and direct team in undertaking clerical audit appropriate to team practice. Actively promote implementation and upholding of Health Authority, District and Trust policies, procedures, guidelines and directives. Actively promote the correct implementation and upholding of relevant statutory measures and governmental directives. Establish and maintain effective relationships within the Trust and with outside statutory and voluntary agencies. This is in order to raise the profile of district nursing and influence, where possible, the ongoing development of patient care and services Set priorities to achieve a balance between patient needs, managerial duties, and staff training within resource limitations. Provide training, undertake work based assessments and supervision of students. Typically, for periods of 8 weeks, at least once a year, students will usually be placed within a team while training for RN Level 1, but other students would receive similar input. Act as preceptor to newly appointed staff and staff undergoing a supervised revision of practice. Encourage and organise team participation in regular clinical supervision. Maintain a high standard of evidence based nursing expertise. Assist the Operational Lead in monitoring clinical levels of expertise employed by the team in order that training needs amongst staff can be identified. Organise and supervise the process of induction for new team members and contribute to the induction of other disciplines, as appropriate. Be pro-active in the promotion of suitable education in care homes, within resource limitations. Identify personal training/educational requirements in conjunction with Clinical Lead and Operational Lead and attain and maintain an agreed level of expertise through ongoing training and development. Maintain a caring environment through counselling and support of colleagues and supervised staff, as necessary. Maintain good industrial relations. Comply with the Safety at Work Act.
