Summary

KEY DUTIES AND RESPONSIBILITIES: Management Provide line management and day to day support to Support Workers. Provide regular supervision, guidance and performance review to a team of Support Workers ensuring all staff are supported and equipped to deliver the service requirements. Support staff to deliver a high standard of care to individuals requiring the Home First service in the community, as directed by their Support Plan. To support the team to ensure that staff are properly trained, developed and skilled to fulfil their roles and maintain competence. Undertake staff appraisals and play an active role in team meetings. To ensure the Support Workers are properly supported and have access to debriefing and support if they have encountered emotionally/contentious/difficult situations whilst working with frail and vulnerable older people. Manage day to day staff attendance and performance, and escalate through the relevant Trust policy as delegated by the Team Leader. Support and assist the Team Leader in recruitment and induction of new staff and associated duties Operational Support the Team Leader to supervise the day to day service ensuring that staff are delivering care within the boundaries of an agreed Support Plan/Care Plan and within the individuals competency framework. Ensuring support plans maximise the efficient use of resources whilst meeting individuals needs. Promote effective communication within the team and with service users to encourage effective and efficient service delivery. Support the Team Leader in responding to service delivery issues, complaints, staffing issues and shortages; resolving speedily to minimise impact on the delivery and service user experience. Contribute and where necessary undertake outcome focused assessments and reviews as part of a multi-disciplinary team. Contribute to complex case reviews and safeguarding case reviews by providing patient specific information to other professionals. Provide personal care to patients as required, delivering care within the boundaries of a provided Support Plan or Care Plan. Undertake first visits to assess patients environment, identifying any risks, transcribe medication and provide personal care as required. Contribute to changes to the Care Plan as requiredf Assist in the management of a smooth and effective transition for service users exiting the in house service provision. Foster effective relationships within the service user, with external partners and communities. Providing robust links between all community services. Work in partnership with the individual, their carer and or representative, other health and social care professionals, external agencies and colleagues to provide a holistic and personalised service as required. Ensure accurate record keeping is maintained in accordance with the Trust policy. Ensure the service is delivered is in line with Trust policies and procedures. Regularly monitor standards and the quality of service delivery through a suite of management information supported by internal and external audit. Assist in the collation of data / feedback of service user satisfaction and review of service performance. Assist with the Health and Safety Audit process and undertake risk assessments as delegated by the Operational Lead as required. Coordinate cover for patient visits for a group of staff as needed. To provide an on call cover service as required by the service. To be flexible to work shifts and provide weekend cover as required by the service within the office and including delivering personal care within the community Ability to travel across the geographical footprint of the Trust in order to respond to the needs of the service. Other responsibilities To work as flexibly within a team and create good relationships with colleagues to assist in the smooth running of the service To participate in the appraisal and personal development process to identify their own needs and how they can be met. To support staff with raising concerns and reporting incidents as per Trust policies. To be aware of and ensure that staff adhere to Midlands Partnership Foundation Trusts Adult Safeguarding Enquiry policy and procedures. Comply with and ensure staff are aware of organisational policies, procedures and legislative frameworks. To work within infection control guidelines to ensure that support given does not constitute a risk of infection either to the worker, or the individual whilst working in the individuals home To be aware and ensure that staff comply with all Trust policy requirements in relation to Information Governance, including the Freedom of Information Act 2000, the Data Protection Act and the Trusts Electronic Record Management Standards & Guidance. To have an understanding of the Care Quality Standards (KLOE) to support the manager with the delivery of the service To be flexible to undertake any other duties that may be allocated from time to time that is appropriate to the position and support the service needs. Systems and equipment To utilise a variety of medium to ensure effective communication between all staff and stakeholders. Utilise electronic systems that support the allocation of work. Decisions and judgements Ability to work under your own initiative with an awareness of own role boundaries. Communication and relationships Communicate effectively using tact and persuasions where they maybe barriers to understanding. Communicate effectively with the wider team and be involved in decision making by providing evidence to support any assessments required to determine potential needs and funding eligibility for long term needs and further ongoing support. Ability to work as part of a team sharing information and records that enables the right decision and best outcomes for the people. Answering calls from operational staff, GPs and other healthcare professionals. To answer and deal with general enquires and telephone calls from the Internal and External stakeholders. Physical demands of the job To provide personal care in sometimes difficult environmental situations. Most challenging/difficult parts of the job Potential exposure to highly pressured situations requiring diplomacy and tact.
