Summary
An exciting opportunity to join the Transfer of Care Integrated Team as a Band 4 Transfer of Care Facilitator. We are a team based at the hospital that support patients discharges from the acute wards and the community bed facility.
The role provides support to wards, attend board rounds and ensure that all patients are discharged safely with the care they require .We support patients to return to their own homes or to 24 hour car setting.
We meet with the executives on a weekly basis to discuss the patients length of stay.
We work Monday to Sunday 8-4pm.
To attend Board rounds
Assess patients for Home First Discharges
Provide patient information for the No Criteria to reside data
Complete equipment assessments to ensure safe discharges.
