JOB PURPOSE
The post holder will work autonomously and in collaboration with other health care professionals, statutory and voluntary agencies, and the planned care locality teams to deliver excellent standards of patient care for patients requiring support in unplanned care.


• To Deliver high quality, skilled care to patients at home.
• Work shift patterns including weekends and bank holidays. The service operates through pre-rostered shift patterns spanning 8AM – 10PM 7 days a week.


• To utilise specialist skills and knowledge to holistically assess needs and provide short term programmes of nursing care for vulnerable adult’s base on evidenced based practice.


• To Work as required on a flexible basis to cover all roles required within the team including.
- Frailty Hospital at Home (H@H)
- Early intervention vehicle (EIV)

• To Work within and across professional and organisational boundaries, liaising with GP’s, other professionals, and voluntary agencies to deliver individualised and expert clinical care to a high standard.


• To support the locality manager/clinical lead and colleagues in delivering high quality care that delivers best value and meets the requirement of commissioners.


• To take responsibility for managing members of the team, including undertaking appraisals with the support of senior staff.
