We are an adult CMHT in Bournemouth. We are all passionate about our jobs and have a warm and friendly atmosphere within the team.
The ideal candidate will work as part of a team, which promotes social inclusion and access for services users in recovery from mental health difficulties.
Our team focus on the individual needs and wishes of service users, working across boundaries of care, organisations in enabling and empowering individuals to access opportunities which may promote their recovery. This activity is coordinated through the Care Programme Approach/Care Management process.
This is a full-time post only.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
You will provide support, give time to an allocated group of service users and thus promote their recovery and maintain them in their community environment. Within the context of this assess, plan, promote and review individualised activity plans utilising team procedures.
You will assist care coordinators in the development of holistic individualised care plans.
You will work with minimal supervision and to manage a caseload of up to 20 clients full time.
You will work within Trust policies and procedures
You will travel within the geographical work area in an effective manner.Driving is an essential criteria of this role. Please ensure you are qualified to drive and do you have access to a vehicle for work purposes.
You will work with potential providers in promoting access to activities in the local community by building good quality sustainable relationships that will support clients in the long term. Promote compassionate communities.
